Students and staff need to ask themselves the following 4 questions daily before leaving home, and if they answer “YES” to any of the questions, you will need to complete the online COVID self-assessment (https://covid-19.ontario.ca/self-assessment/) and follow-up with your local Public Health Unit.

| 3. Have you had close contact with a confirmed case of COVID-19 without wearing appropriate personal protective equipment? “YES” or “NO” |

Do you have any of the following symptoms: “YES” or “NO” * Fever *New Onset of Cough *Worsening Chronic Cough * Shortness of Breath * Difficulty of Breathing * Sore Throat * Difficulty Swallowing * Decrease or Loss of Sense of Taste or Smell * Chills * Headaches * Unexplained Fatigue/Malaise/Muscle Aches (Myalgias) * Nausea/Vomiting, Diarrhea, Abdominal Pain * Pink Eye (Conjunctivitis) * Runny Nose/Nasal Congestion Without Other Known Cause |
